Recovery Management



Intensive Recovery Management Personalized support to help individuals stay on track with their goals and recovery journey. Service Coordination Streamlined collaboration among providers, ensuring access to the full range of behavioral health and support services. Linkage to Community Resources Connection to housing, employment, education, transportation, and other essential community-based services. Monitoring of HCBS-AMH Services Oversight to ensure that Home and Community-Based Services for Adults with Mental Health needs (HCBS-AMH) are delivered effectively, appropriately, and in alignment with the person’s goals.
Client Support/ Service Coordination



SHCS plays a key role in supporting clients through direct assistance, coordination of services, and ongoing quality assurance. Core responsibilities include: Assisting with Benefits and Entitlements Support clients in completing applications for public assistance, health coverage, and entitlement programs to ensure access to essential resources and support. Addressing Service Delivery Issues Respond promptly and effectively to any challenges or concerns that arise during the course of service delivery, ensuring continuity of care and client satisfaction. Appointment Scheduling and Administrative Support Coordinate client appointments, manage phone communications, and maintain accurate and up-to-date service records in compliance with organizational and regulatory standards. Service Monitoring and Updates Track the availability, status, and effectiveness of services being provided. Stay informed of new services introduced or existing services that are being modified or discontinued. Quality Evaluation and Continuous Improvement Assess the overall quality of services on an ongoing basis, identifying gaps or areas in need of improvement and contributing to strategies that enhance service outcomes.

Clinical & Behavioral Health Services Community Psychiatric Supports and Treatment (CPST) Individualized, community-based psychiatric rehabilitation and skill development services. Psychosocial Rehabilitation (PSR) Recovery-focused services that help individuals develop social skills, manage symptoms, and achieve greater independence. Substance Use Disorder Services Treatment and recovery supports for individuals experiencing substance use challenges. Recovery & Life Skills Support Employment Services Job readiness training, placement assistance, and ongoing vocational support. Flexible Funds Limited financial support to assist with non-recurring needs that help maintain stability and promote recovery.
Monitoring Services of the HCBS-AMH Program

Housing Services are designed to promote independence, stability, and recovery for individuals living with mental health conditions. We combine safe, affordable housing with coordinated, person-centered support to help clients thrive in the community. Core Housing Supports 🏠 Supported Housing Services Clients are placed in a range of housing settings where they may live independently or with others who have similar mental health experiences. Housing environments are tailored to individual needs and preferences, fostering a sense of community and belonging. 🚨 24-Hour Crisis Support Services Round-the-clock access to crisis support ensures immediate assistance during emergencies, helping prevent hospitalizations and ensuring safety and stability in the home. 👥 Personal Care Services Assistance with daily living activities such as bathing, grooming, meal preparation, and medication reminders, provided in the comfort of the client’s home. Integrated Behavioral Health & Clinical Services Behavioral Health Care Services Onsite or mobile mental health care, including therapy, psychiatric services, and symptom management. Therapist Support Licensed therapists provide individual, group, or family counseling to support emotional well-being and promote recovery. Registered Nurse (RN) RNs offer health monitoring, medication administration, and coordination with primary care providers to manage chronic conditions. Psychosocial Rehabilitation Specialist Professionals work with clients on skill development, social integration, and building community participation. Coordinated Support Services Intensive Case Management A highly effective approach that combines individualized care planning with proactive coordination of medical, behavioral, and social services. Case Management Dedicated case managers help navigate housing, health care, benefits, and community resources while advocating for client needs. Transportation Services Reliable transportation to and from medical appointments, community events, and essential services to support independent living and wellness.
